After receiving the claim we, Colonnade Insurance S.A. Branch in Poland (Insurance Company), begin the process of its verification. In case of a complete application, we provide a decision immediately, not later than within 30 days from the receipt of the claim.
In case it is not possible to provide a decision within the above deadline, the Insurance Company shall immediately inform the insured person about the obligation to complete a claim or to provide explanations necessary to verify a claim. In this case, the Insurance Company shall issue a decision within 14 days from receiving the last missing document/information.

Individual client
Claim form personal accident
In order to enable the claim to be verified as quickly as possible, it is necessary to attach the following documentation:
- copies of all medical documents connected with the accident, including documents confirming completion of medical treatment and rehabilitation
- police report (if applicable)
- copy of the insurance policy/certificate
If the claim concerns the death of the Insured, additionally:
- medical documents confirming the cause of death
- prosecution / coroner report
- autopsy report (if autopsy was conducted)
- statement on establishing the Beneficiary
- marriage certificate or birth certificate (if the Beneficiary is a descendant or spouse of the Insured)
- notarized copy of the Beneficiary’s identity card
- original death certificate or notarized copy
Claim form trip delay/baggage delay/baggage lost
In order to enable the claim to be verified as quickly as possible, it is necessary to attach the following documentation:
- original receipts/invoices for incurred costs / lost items
- notification to the carrier together with the decision on granting/refusing to award damages
- police reports (if any)
Claim form trip cancellation
In order to enable the claim to be verified as quickly as possible, it is necessary to attach the following documentation:
- ticket for the insured trip
- confirmation of trip cancellation notification filed to travel operator/carrier
- information on amount of return awarded from the travel operator/carrier due to cancellation
- documentation confirming the reason of trip cancellation (medical documentation, police report)
Claim form Medical costs reimbursement
In order to enable the claim to be verified as quickly as possible, it is necessary to attach the following documentation:
- original medical bills
- confirmation of medical diagnosis from a doctor
- police reports (if applicable)

Claim form group travel personal accident
In order to enable the claim to be verified as quickly as possible, it is necessary to attach the following documentation:
- copy / scan of full medical documentation related to the accident (including copy of the hospital discharge summary and confirmation of completion of the treatment and recovery process)
- copy of police reports and results for alcohol content in blood of the Insured (if applicable)
If the claim refers to the death of the insured, the following should be attached:
- medical documentation confirming cause of death
- prosecutor’s report
- results of the autopsy (if such took place)
- declaration on the beneficiary authorization
- copy of marriage certificate or birth certificate (if the beneficiary is the offspring or spouse of the insured)
- notary certification of the ID copy of the beneficiary
- original of the death certificate or its notary certification
Claim form delay / loss of luggage, flight delay
In order to enable the claim to be verified as quickly as possible, it is necessary to attach the following documentation:
- original receipts/invoices for incurred costs/lost items
- copies of flight tickets/online reservations of the original itinerary
- copy of the carrier’s confirmation the event took place along with the decision of granting/refusing to award the damages
- other documents confirming the inability of taking the trip
Claim form return of costs of medical treatment
In order to enable the claim to be verified as quickly as possible, it is necessary to attach the following documentation:
- original receipts for the medical expenses incurred
- copy of medical documentation including the diagnosis
- copies of police reports (if available)
Claim form loss of personal property
In order to enable the claim to be verified as quickly as possible, it is necessary to attach the following documentation:
- a police report
- documents confirming the ownership of the lost property (invoices, guarantees, etc.)
- other documentation confirming the loss/damage of the property

A scan will be sufficient unless originals during the claims process will be requested.
If you report online or via szkody@colonnade.pl, an email is sent upon submission confirming that the claim has been registered. If the applicant does not receive an email within 48 hours, please contact us.
